In the Fitchburg Police Department’s strive to provide professional comprehensive service, we are pleased to announce our involvement with the Alzheimer’s Support Network of South Central Massachusetts, Inc. In a combined effort, the Fitchburg Police Department offers ALZHEIMER'S ALERT to the citizens of our community.

Registration is voluntary and to participate, all that is required is for the ALZHEIMER'S ALERT Form to be completed and returned to the Fitchburg Police Department where it will be kept on file. The form may be acquired on this web site (see below for download) or in the lobby of the Fitchburg Police Department. Information requested on the form includes the patient’s name and address, a physical description and a recent photograph. This information will be kept in strict confidence. Contact numbers for family members and/or neighbors, if appropriate, are also requested.
